The employer is contesting a two-item serious citation and a penalty of $24,020. The serious citation includes the alleged violations of 1910.147(c)(4)(i), for failure to develop, document, and utilize procedures for the control of potentially hazardous energy when employees were engaged in activities covered by this section; and 1910.178(m)(10) for failure to ensure a load backrest extension shall be used whenever necessary to minimize the possibility of the load or part of it from falling rearward (No. 18-2033).
