The employer is contesting a four-item serious citation and a penalty of $26,000. The serious citation includes alleged violations of 1910.147(c)(4)(i), for failure to develop, document and utilize procedures for the control of potentially hazardous energy when employees were engaged in activities covered by this section; 1910.145(c)(5)(ii)(D), for failure of lockout/tagout devices to indicate the identity of the employee applying the devices; and 1910.147(c)(6)(i), for failure to conduct an annual or more frequent inspection of energy-control procedures to ensure the procedures and requirements of this standard were followed (16-1423).
