The Occupational Safety and Health Administration announced April 19 it has proposed penalties of $127,350 against a Wisconsin packaging printing and laminating company following a worker’s death.
OSHA said it began its inspection of American Packaging Corp. in Columbus, Wis., last October after learning that a maintenance technician was killed during an explosion at the plant. Flammable vapors were present when the worker was using a grinder to cut off a bolt while installing a metal floor threshold, the agency said.
