OSHA has issued Sumitomo Bakelite North American Inc. $171,884 in proposed penalties and citations alleging one willful violation and one serious violation involving respiratory and fire hazards at its facility in Kenton, Ohio, the Department of Labor said Thursday.
The company, operating as Duez Corp., allegedly failed to ensure the HVAC system effectively maintained a positive pressure on the control room and automatically shut off in the event of a formaldehyde release, an inspection from the Occupational Safety and Health Administration found.
