The employer is contesting a two-item serious citation and a penalty of $15,144. The serious citations include the alleged violations of 1910.147(c)(7)(i)(A), for failure to provide training for authorized employees in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace and the methods and means necessary for energy isolation and control; and 1910.147(d)(2), for failure to ensure that machines or equipment were turned off or shut down using the procedures established for the machine or equipment (17-0067).
