The employer is contesting a serious citation in two parts and a penalty of $8,908 for the alleged violation of 1926.502(d)(15)(i), for failure to use anchorages in the attachment of personal fall arrest equipment that were capable of supporting at least 5,000 pounds per employee attached; and 1926.502(d)(15)(ii), for failure to ensure that anchorages used for attachment of personal fall arrest equipment were designed, installed and used under the supervision of a qualified person when such equipment couldn’t be independent of any anchorage being used to support or suspend platforms (16-2048).
