The administrative law judge affirmed and vacated various items filed against Clean Fuels of Indiana Inc. and assessed an $11,600 penalty following a fatal accident at an under-construction retail gas station in Daytona Beach, Fla.

The Occupational Safety and Health Administration conducted an inspection of the worksite after a Clean Fuels lead technician found the side technician unresponsive at the bottom of a well that contained a submersible turbine pump.
