BP Products North America Inc. has been cited with 10 serious violations and one other-than-serious violation and faces proposed penalties of $156,250 after two workers suffered fatal burns in an explosion at an Oregon, Ohio, refinery last year, the US Labor Department said Thursday.
While workers were correcting the rising liquid levels in the fuel gas mix drum, a flammable vapor cloud ignited and caused the explosion, the agency said.
Inspectors with the department’s Occupational Safety and Health Administration found that the company allegedly failed to meet process safety management procedures, according to the DOL.
