3M Hit With $312,000 Fine Over Wisconsin Worker’s Fatal Injuries

3M Company has been cited with two willful safety violations and proposed penalties of $312,518 after a worker fatality at a Prairie du Chien, Wis., facility, according to the Labor Department Tuesday.

Occupational Safety and Health Administration inspectors found that in May while helping to set-up a plastic extrusion line, a worker became caught in a machine’s rotating rollers and later died from their injuries.

OSHA cited the manufacturer for failure to use procedures for the control of hazardous energy, and failure to implement energy control application steps when employees set up the production line, said the agency.
