Davita Nurse Collective Consent Forms Must Be Redone, Court Says

A former DaVita Inc. nurse needs to send out new consent forms to opt-in plaintiffs interested in joining her proposed wage collective, a federal court ruled based on the recommendation of a magistrate judge.

Anduin Lightner’s case will be closed until she redistributes corrected consent forms that are approved by the court, Judge Nina Y. Wang, of the US District Court for the District of Colorado, said in a Wednesday order denying the health-care provider’s motion to strike the forms.
