NEW YORK—Two Oxford health insurance companies have been fined $665,000 for failing to provide members with explanation of benefits (EOB) forms or telling them how to challenge claim denials, the state Department of Financial Services announced Sept. 20.
In a settlement with DFS, the two Oxford Health companies—Oxford Health Plans (NY) Inc. and Oxford Health Insurance Inc.—also agreed to take steps to revise procedures for issuing EOBs and conducting utilization reviews. Oxford Health is owned by UnitedHealth Group.
