Health plans are becoming more aggressive in trying to recover millions of dollars they say health-care providers have fraudulently billed.
A host of Anthem Inc. subsidiaries operating as Blue Cross and Blue Shield plans across the U.S. sued a Georgia provider that, they say, has been billing laboratory tests as if they had been performed in a hospital. An outside laboratory performed the tests, which meant the plans would have either disallowed the claims or reimbursed the costs at a lower rate.
