Bacilio Rios Almanza was cited and fined $262,174 for failing to protect employees from fatal fall hazards while working on a residential roof in Appleton, Wis., in 2024, resulting in two willful violations and one serious violation, the Occupational Safety and Health Administration announced Monday.
OSHA said that Almanza did not provide fall protection equipment, failed to train workers on fall protection, and did not ensure workers wore hard hats. This is the 10th time OSHA inspected Almanza’s work sites and cited the contractor for similar violations.
