ST. LOUIS—Tyson Foods Inc. was cited for seven alleged safety violations relating to ineffective equipment inspections and lockout/tagout hazards after a mechanic was fatally injured in March 2012, the Department of Labor’s Occupational Safety and Health Administration said Aug. 28.
According to an OSHA statement, the mechanic at a Nebraska beef-processing facility was killed while performing maintenance work beneath a piece of equipment that had been secured by a chain and quick link. The chain failed, and the equipment crushed the mechanic.
