Ohio Double Fatality Leads to One of Largest OSHA Fines in Years

Federal workplace safety regulators proposed $1.57 million in fines against an Ohio company that specializes in cleaning large storage tanks after an accident that claimed the lives of two workers in February prompted an on-site inspection.

The penalty levied against Great Lakes Tank and Vessel L.L.C. of Cleveland is the fifth-highest the Occupational Safety and Health Administration has issued since January 2015—and included a fine for not evaluating the effectiveness of respirators after an attempt had been made to repair them “with tape due to damage” from chemical exposure.
