The owner of a Wisconsin corn mill where a grain dust explosion killed five workers failed to implement suggested, meaningful changes to its operations at the facility, the agency charged with investigating chemical accidents says.
The Chemical Safety and Hazard Investigation Board made nine safety recommendations to Didion Milling Inc. following the explosion that also injured more than a dozen workers in 2017. The CSB said Monday the company hasn’t implemented any of them, including hiring a third party to perform risk assessments and developing comprehensive combustible dust process safety management systems.
