Chemical Safety Board Issues Two Final Investigation Reports

The U.S. Chemical Safety Board outlined fatal industrial workplace failures including improper mixing of chemicals and hot work safety failures in a pair of final investigative reports made public on Friday.

The white papers cap the agencies’ review of a May 2019 hydrogen-fueled blast that killed four workers at AB Specialty Silicones LLC in Waukegan, Ill., and the September 2020 fire at the Evergreen Packaging paper mill in Canton, N.C., that killed two contractors.

AB Specialty Silicones makes silicone-based raw materials, vinyl polymers, and resins for the cosmetics, dental, and medical industries and other uses, its website says.
