The California Division of Occupational Safety & Health cited Chevron USA Inc. for 25 alleged violations related to the August 2012 fire at the company’s refinery in Richmond, Calif., and proposed nearly $1 million in fines, the agency announced Jan. 30.
Cal/OSHA said the penalties are the highest in its history and the maximum allowed under state law for the citations issued.
