Trench Death Brings $157K Fine for Pennsylvania Construction Firm

A Pennsylvania construction company faces a proposed $157,169 fine in connection with an incident in which a worker died in a trench collapse.

The Occupational Health and Safety Administration issued citations Nov. 17 to R. A. Monzo Construction Co., based in Latrobe, Pa., following an inspection after the accident at a construction site on May 22.

The citation claims the company committed one alleged willful violation when a superintendent watched workers installing a new storm sewer in an “unprotected” nine-foot trench. The trench collapsed on one worker, killing him, which led to the proposed $126,749 portion of the OSHA ...
