Texas Firm Fined $497,920 For Workers’ Deaths from Toxic Fumes

A Pasadena, Texas, firm faces nearly a half million dollars in Occupational Safety and Health Administration fines for exposing workers to confined space hazards after two workers died while cleaning a tank trailer.

OSHA investigators determined that Alpha Technical Services —doing business as Quala Rail and Specialty— exposed workers to confined space hazards after two employees were killed by fumes while cleaning a tank trailer in December 2019. The industrial cleaning and maintenance company faces $497,920 in penalties.
