Fatal Trench Collapse in Missouri Leads to OSHA Safety Citation

Brown Construction Co. Inc. has been cited for four serious violations and proposed penalties of $58,008 by the US Department of Labor’s Occupational Safety and Health Administration after a fatal Dudley, Mo., trench collapse, according to the agency Wednesday.

A worker installing storm water drainage in an 8-foot-deep trench along Old Highway 60 in Dudley, Mo., died when the trench collapsed. OSHA determined Brown Construction Co. Inc. failed to use a trench box, or to shore or slope the trench walls to prevent collapse.
