Ex-Mine Safety Manager Sentenced Over Failure to Report Accident

A former mine safety manager has been sentenced to two years of probation and fined $2,000 for conspiring to make false reports about a mine accident that resulted in an employee’s partial finger amputation, the Justice Department said Tuesday.

Curtis Floyd of Billings, Mont., who worked at Signal Peak Energy LLC, failed to complete or submit a Department of Labor Mine Safety and Health Administration Mine Accident, Injury and Illness report after a worker’s fingers were crushed while loading materials.
