A Massachusetts company faces $1.4 million in fines for 18 separate willful, repeat and serious OSHA violations in connection with a trench collapse that killed two workers in October 2016.
Robert Higgins and Kelvin Mattocks died on Oct. 21 in Boston when the approximately 12-foot-deep trench in which they were working collapsed, breaking an adjacent fire hydrant supply line and filling the trench with water in a matter of seconds, according to a press release from the Occupational Safety and Health Administration.
