Arkansas Contractor Cited by OSHA After Workers’ Sewer Deaths

Belt Construction Inc. has been cited for six serious and two willful violations and proposed penalties of $287,150 after two workers suffered fatal injuries at a work site in Edmund, Okla., according to the Department of Labor Wednesday.

The company allegedly failed to test oxygen levels in a sewer before two workers entered and later died because of a lack of oxygen, said the agency.

The Occupational Safety and Health Administration cited Arkansas-based Belt Construction for failure to conduct pre-entry planning, provide rescue equipment, and train workers on confined space entry, DOL said.
